The Team Health Assessment.

A team instrument, not an individual one. It reads the group as a unit — one team, answered by the people in it — and belongs to the Team Effectiveness pathway.

Fifteen statements, rated usually, sometimes, or rarely. They resolve into the five tiers of Patrick Lencioni's team pyramid — trust, conflict, commitment, accountability, results — and the report places you on that pyramid at the lowest tier that is not yet healthy. That tier is the constraint every tier above it inherits.

It takes about ten minutes. You receive a branded summary report with your tier scores, your placement, the two tiers carrying the most risk, and the Compass discipline that rebuilds each one. It is open to anyone — no password, no request step — and your answers stay in your browser unless you choose to email the report.
